Annual Golf Outing - SOLD OUT

Please join us for a fun day of golf and food on Saturday, June 18, 2011!

  You are invited to attend the Portage Lakes Fireworks Association's Annual Golf Outing to be held at Turkeyfoot Golf Course on Saturday, June 18, 2011, teeing off at 1:00 p.m. The outing includes 18 holes of golf, lunch at the turn and dinner after the outing at Dusty's.

The cost of the golf outing is $115.00 per person if registered and paid in full by June 4, 2011. After June 4, 2011 the cost of the outing is $140.00 per person. There will be no exceptions. We can only accept 36 teams (144 golfers) so register early to hold your spot.

Live band starts at 6:00 p.m. and dinner buffet starts at 7:00 p.m. The awards ceremony for the Annual Golf Outing will commence at 8:30 p.m. when the winners will be announced. Cost for the dinner only is $45.00 per person, with all proceeds going toward the July 3rd Fireworks show. The deadline for the dinner reservations is Friday, June 17, 2011.

You may also contribute by being a participant in the Annual Golf Outing or by choosing one of four ways to be a sponsor. If you are interested in participating in this special event, bring this form, along with a check or money order, payable to the Portage Lakes Fireworks Association (PLFA). All you have to do from that point is show up on the day of the event.
